Long Term Care: Home Based and Community Services

Most people when they think of long-term care think of care in a nursing home. The majority of people only enter a nursing home when their needs can no longer be met outside of a treatment facility. Home based and community services are a range of services that allow people to remain in their homes and retain their independence for as long as possible.
The following is a list of some of the common types of Home based and community services:

Adult Day Service Programs – is sometimes called adult daycare depending on the functional level of the person needing the service. It provides social interaction and allows family caregivers to continue to work. Most operate on a five-day workweek schedule.

Emergency Response Systems – provide automatic emergency response through electronic monitoring. Persons that live alone wear a signaling device that you activate when needed.

Homemaker/Chore Services – provide help with routine household chores such as food preparation, housekeeping and heavy household chores such as washing floors, windows or shoveling snow.

Respite Care – provides relief for caregivers who must provide total care for family members who cannot care for themselves. Care can be given in a nursing home, residential care facility or the persons home.

Transportation Services – provide transport to and from doctor’s appointments, shopping centers and other community services.

Meals Programs – such as Meals on Wheels provides food for persons living on their own and sometimes in community settings.

Senior Centers – provide information and referral to resources available to seniors as well as educational, social, nutritional, and recreational services.

Friendly Visitor/Companion Services – volunteers who visit persons who live alone or maybe frail; consist of short visits of two hours or less on a regular basis.
