- Hospital Errors
When hospital errors occur, they are only reported 13% of the time. Often, hospital staff may not recognize that an error harmed a patient or may not realize that harmful events should be reported. A recent report analyzed nearly 300 adverse patient events based upon a review of medical records. A review was conducted to see if the respective hospitals had identified any medical error and informed the patients. The report concluded that most often the hospital did not do so.
